ATTORNEY'S REQUEST FOR NAME CHANGE

The ARDC master roll of registered attorneys lists a lawyer's name in the same manner as the name appears on the roll of attorneys. The roll of attorneys is a list of all attorneys admitted to practice of law in Illinois. The roll of attorneys includes the names of lawyers, even if the lawyers have not registered with the ARDC. The roll of attorneys is maintained by the Clerk of the Illinois Supreme Court. A lawyer's name on the Clerk's roll of attorneys is the name under which the lawyer was admitted to the practice of law in Illinois or, if any name changes have been allowed by the Court, the most recent such change.

In order to change your name on the ARDC master roll, you must file a motion with Clerk of the Supreme Court to change your name on the roll of attorneys. The motion may be in the form of a formal letter. The motion should include the name under which you are listed on the Clerk's roll of attorneys (usually, the name under which you were originally admitted) and the name you would like to use. If your name change results from a marriage, attach a copy of your marriage license. If your name change results from a dissolution of marriage, attach a copy of the dissolution judgment.  While neither copy needs to be certified, it must be a copy of a certified copy. No fee is required.

You need not serve ARDC with a copy of the motion. Once the Court has changed your name, the Supreme Court Clerk will notify you and ARDC. The Clerk will then change your name on the ARDC master roll of registered attorneys and the ARDC will send you a new identification card at your registered business address.
