DUPLICATE IDENTIFICATION CARD
A request for a duplicate identification card must be in writing. Mail or deliver the request to:

You may also send your request by facsimile to ARDC at (312) 565-0997. Upon receipt of the request, the registration department will issue and send you a duplicate identification card at your registered business address.
