For identification purposes, we request that you include your name in full, your ARDC number, your birth date, and your social security number. Identify the reason for your request in the Reason for Request section. Sign and mail or fax the completed form to our Registration Department at our Chicago office at:
In most instances, our verifications are limited to public information regarding a lawyer’s registration and disciplinary history. Public information includes the lawyer’s full name as licensed, the date of the lawyer’s admission, the lawyer’s current registration status, and any public disciplinary information (generally, formal disciplinary prosecutions or sanctions). In these instances, verifications will not disclose investigations and other information made confidential by Supreme Court Rule 766. Pursuant to authorization from the Supreme Court under Rule 766(b)(2), the written verification will also disclose pending confidential matters if the Request is for a verification for use by one of the following: admission authorities in another jurisdiction in connection with an application for admission to the bar (but not isolated case/ pro hac vice admission); a screening committee in connection with an application for a judgeship; or a governmental employer in connection with an application for government employment. If your request is for one of the three reasons enumerated above, fill in the name of the entity to which the verification will be provided on the Reason for Request section. Some courts and offices require that verification of a lawyer’s status be provided by the Clerk of the Supreme Court of Illinois rather than by this Commission. These courts and offices include: The Unites States Supreme Court, The United States Tax Court, the United States Patent and Trademark Office and the Bar of the District of Columbia.
